On-line form
The On-line form web part allows live site users to fill in and submit a pre-defined on-line form. This is typically useful in cases when you need your website users to provide structured information such as orders, questionaires, satisfaction surveys, etc. Forms can be defined in the administration interface through the Forms application. The same interface can be used to view and manage submitted form records.
